It will implement a redesigned sector neutral setup that will move a significant part of the traditional health activities to the patient’s own home. The new system will monitor changes in the citizen’s conditionthat may need actions without delay to keep him/her as close to a normal life as possible, supported by clinicians in a new Shared Response and Coordination Centre run by aboard with members from municipalities and the region. The sector neutral clinical solution will be supported by economic incentives in a bundled care financing model and the outcome will be documented by research.
