Person-centric Health Innovation: New Service Models for Preventive Care Delivery to Elderly and Citizens with Chronical Conditions or at Risk of Getting Chronical Conditions (PreCare Phase II )


Abstract:

The project will make a national roadmap for a major paradigmshift in health care. It will implement a redesigned sector neutral setup that will move a significant part of the traditional health activities to the patient’s own home.The new system will monitor changes in the citizen’s conditionthat may need actions without delay to keep him/her as close to a normal life as possible, supported by clinicians in a new Shared Response and Coordination Centre run by aboard with members from municipalities and the region.The sector neutral clinical solution will be supported by economic incentives in a bundled care financing model and the outcome will be documented by research.
